The form is easy to fill in but it did take quite a bit of time to find it - perhaps it could be an option on your homepage?
Thought it was an easy form to follow, much better than using the phone. When i rang up the line cut me off with your out of hours message even though it was about 12:00 on a Thursday. never mind though because the web page is good.
I think it is very good. It is simple and easy to follow. I can't think of any further suggestions.
It would have been nice to be able to navigate between pages, without having to go through each page.
